Holmes beschließt, der Sache nachzugehen – und so führt die Reise Holmes, Dr. Watson und Alice über Europa bis zur Küste von Kreta.

Nach einer langen Reise erreichen Sherlock Holmes, Dr. Watson und Alice endlich die abgelegene Leprainsel Spinalonga vor der Küste Kretas. Was als Suche nach dem verschwundenen englischen Arzt Dr. Thompson beginnt, wird schnell zu einer Begegnung mit einer Welt, die von Krankheit, Angst und Vergessen geprägt ist. Während Watson als Mediziner mit dem menschlichen Leid ringt, beobachtet Holmes die Insel mit kühler Präzision.

Holmes hat die Ermittlungen zu dem Mordfall übernommen, während Dr. Watson feststellen muss, dass eine größere Menge Phenol aus dem Medikamentenlager entnommen wurde. Holmes und Watson setzen umgehend Schwester Mary darüber in Kenntnis. Alice hingegen kämpft nach wie vor mit ihren eigenen Dämonen und findet in einem jungen, gutaussehenden Leprakranken eine willkommene Ablenkung.
