Imagine walking into a negotiation to sell your house. The buyer knows what every comparable house in the neighborhood sold for, but you don't. That's essentially how many physicians have negotiated insurance contracts for decades. The insurance companies had the data. We had our contracts, our collections, and maybe a few conversations with colleagues. But now, the rules have changed.

Today we're talking about something that could fundamentally change the way your medical group negotiates with insurance companies: healthcare price transparency.

And here's the really interesting part. The information you need to negotiate better contracts may already be publicly available. The question is whether you know where to find it, how to interpret it, and most importantly, how to use it.

For years, I've watched physicians work incredibly hard to grow their practices, improve productivity, and manage expenses, while overlooking one of the biggest opportunities to improve their financial performance: getting paid appropriately for the work they're already doing.

What happens when the business of medicine begins to interfere with the practice of medicine?

Physicians experiencing exhaustion, frustration, and disillusionment are often handed a familiar prescription: build resilience. Practice mindfulness. Take better care of yourself.

But what if the physician isn’t the problem?

What if a perfectly reasonable human being is having a perfectly reasonable response to a healthcare system that increasingly puts financial incentives, productivity demands, and organizational priorities at odds with good patient care?

In this episode of Medical Money Matters, I sit down with Dr. MaryAnn Wilbur, a physician, public health strategist, author, researcher, and advocate who is working to change the systems in which physicians and patients find themselves.

MaryAnn trained and practiced as a gynecologic oncologist before stepping away from clinical practice in 2022. Her experience led her to examine a much bigger question: Why are physicians leaving medicine, and what does their departure tell us about the health of the healthcare system itself?

MaryAnn is co-author of The Doctor Is No Longer In: Conversations with U.S. Physicians, Director and CEO of Health Equity Consulting, and an executive producer of Suck It Up, Buttercup, a documentary examining systemic problems in American healthcare. She is also involved with Medicine Forward and the Clinician Burnout Foundation.

One of the questions at the heart of this episode is one I suspect many physicians have quietly asked themselves:

“Is it me—or is there something fundamentally wrong with the system I'm working in?”

That question deserves more than a wellness seminar.

It deserves an examination of the systems, incentives, economics, and power structures shaping modern medicine.

What would it take to build a healthcare system where physicians and patients can actually thrive?

In this episode of Medical Money Matters, Jill Arena sits down with Dr. Gabe Charbonneau, a rural family physician, technology entrepreneur, nationally recognized advocate for physician well-being, and co-founder of Medicine Forward, a grassroots organization focused on protecting and strengthening the patient-physician relationship. https://medicineforward.org/

His work has taken him from rural family medicine to healthcare technology, including co-founding the EHR automation company Fluent Systems, teaching physicians better EHR workflows, and advising organizations working with AI and medical documentation.

Jill and Gabe also serve together on the board of Orchid Health, an innovative rural health organization in Oregon.

Physician burnout is often framed as a problem of individual resilience: exercise more, meditate, develop better coping skills.

But what happens when the environment itself is the problem?

Jill and Gabe explore a different way of thinking about physician well-being—one that examines the systems, incentives, technology, leadership, and culture surrounding physicians.

Today is a special one, because we’re celebrating 200 episodes together.

When we started this podcast back in September of 2022, the very first episode was about what they don’t teach you in medical school. That idea has really been the thread running through everything we’ve done since. Physicians receive extraordinary clinical training, but then we ask you to lead people, understand financial statements, negotiate contracts, manage multimillion-dollar organizations, oversee revenue cycles, hire and fire, develop strategy, evaluate technology, manage risk, and somehow anticipate what’s coming next in healthcare. And very little—if any--of that was part of your medical education.

So we set out to help fill that gap.

Two hundred episodes later, I’m incredibly grateful that so many of you have come along for the ride. Some of you have been listening almost from the beginning. Some of you found us through a particular episode because you were trying to solve a problem in your practice. Others have joined more recently and started working your way through the back catalog. However you got here, thank you. Truly. You’ve given us your time, your attention, your questions, and, in many cases, your trust. That means a great deal to me and to all of us at Health e Practices.
