  • Can a High-Drop Running Shoe Be Causing Your Patient’s Leaking?
    2026/03/12

Dr. Becky Allen answers a DM question about whether higher drop shoes can contribute to urinary leaking, arguing that they can by altering gait biomechanics from the ground up. Shoe drop is defined as the heel-to-toe height difference inside the shoe, with high heels as an extreme high drop and flat flip-flops as zero drop. A higher drop places the foot in relative plantarflexion, limiting dorsiflexion and midfoot pronation, which reduces the normal internal-rotation cascade up the kinetic chain (subtalar motion, tibial and femoral internal rotation) and leaves the pelvis more externally rotated. With less pelvic internal rotation, the pelvic outlet stays smaller and the pelvic floor cannot lengthen and re-ascend automatically to manage pressure, potentially worsening leaking during walking or running. Clinically, Dr. Becky urges assessing footwear, midfoot mobility, dorsiflexion, and rotation mechanics, and suggests working runners from ~10 mm drop toward low/zero drop (e.g., 4 mm and under).

  • The Entrepreneur Identity Shift: Risk, Fear, and Staying in the Arena
    2026/03/05
In this episode of From the Ground Up, Dr. Becky Allen shares lessons from a walk-and-talk call with her little brother as he prepares to leave a stable job and start his own business. She outlines four takeaways for new and seasoned entrepreneurs: (1) fear is not a red light—fear signals risk and potential, and success can be proportional to the fear and risk involved; (2) most people don’t even start—quitting a job, investing savings, and betting on yourself already puts you ahead of most people; (3) you can’t fail if you stay in the game—failure is quitting because you believe it’s your only option, while staying in the arena means learning, pivoting, and trying again, referencing ideas from Simon Sinek’s The Infinite Game and Alex Hormozi’s focus on “staying in the game”; and (4) vision is necessary but clarity sells—her brother’s long-term vision is commercial land clearing, but she advises starting with one clear offer and message, focusing on an ideal customer and the outcome they want, using the “flashlight vs. laser” analogy. She closes by encouraging listeners to take the leap, share the episode with others, and reach out on Instagram for support.(00:04) - Welcome and Setup(01:49) - Four Takeaways Preview(02:45) - Fear Is Not a Red Light(05:29) - Most People Never Start(09:09) - Stay in the Game(16:24) - Vision Versus Clarity(24:39) - Recap and Closing RemarksJoin the waitlist for the next cohort of Becky’s course: How to Treat the Pelvic Floor Without Kegels: https://www.thepelvicpowerhouse.com/treatingthepelvicfloor. • Stop Managing Pressure. Start Fixing It. A Better Way to Treat Pelvic Floor Dysfunction
    2026/02/26

Dr. Becky Allen argues that traditional pelvic floor courses teach “pressure management” the wrong way by focusing on pelvic floor squeezing, Kegels, and pelvic elevation (e.g., bridges) to manage symptoms rather than addressing why pressure symptoms occur. Using a tube-of-toothpaste analogy, she explains that tightening the “lid” (pelvic floor contractions) doesn’t fix excessive downward pressure and may fail if the force is high enough. She presents a framework based on the formula Pressure = Force/Area, emphasizing that to reduce pressure clinicians should decrease force and/or increase the area the force acts upon. She outlines three ways to decrease force: reduce the load (only when appropriate for healing timelines or injury), displace/distribute force by creating more space above the pelvis via rib cage and thorax expansion, and improve force transfer and absorption through the ground-up kinetic chain (midfoot pronation/supination control, ankle dorsiflexion, tibial/femoral internal rotation, and controlled internal rotation throughout). For increasing area, she describes that many patients have a shortened pelvic floor (often posteriorly) and need pelvic floor lengthening—especially under load—which requires pelvic and kinetic-chain internal rotation. She encourages shifting from symptom management to pressure manipulation for lasting resolution and promotes her course waitlist opening February 23rd, which teaches this full framework with modules (foot, ankle, pelvis biomechanics, rib cage/thorax), 70+ exercise videos, and manual therapy techniques.

  • Pelvic Floor Tension vs Weakness: Why This Model Is Failing Your Patients
    2026/02/19

Dr. Becky challenges the common pelvic floor therapy model that sorts patients into “weak/lengthened” versus “tight/tense/shortened” buckets, arguing these are symptoms rather than meaningful diagnoses or root causes. She explains how this binary approach can keep patients stuck with partial improvements or lifelong symptom-management tools and notes that tightness and weakness can coexist. The episode critiques relying on Kegels—both in traditional strengthening plans and in the “kill the Kegels” movement—stating Kegels don’t train automatic, functional pelvic floor behavior during real-life tasks. Using examples like stress incontinence and dyspareunia, the host emphasizes asking why the pelvic floor is compensating and recommends “getting out of the pelvis” to assess broader drivers such as pressure management from the rib cage/abdomen, pelvis biomechanics, foot/ankle function and the kinetic chain, nervous system/limbic influences, gut issues, and habits, posture, loads, and training. Dr. Becky introduces the ROOT acronym: remember to find the Root cause; Observe the patient’s story, symptoms, goals, and functional movement; Optimize the identified system limitation to restore automatic pelvic floor function; and Transform by building capacity and resilience through functional movement and loading. She encourages clinicians to pick a patient and identify the system driver behind pelvic floor tension or weakness, and promotes the No Kegels course opening to the waitlist on February 23rd.

  • Moving Beyond 80% Function for Life-Changing Pelvic Health Results
    2026/01/29

Pelvic health leaders, how might defining success as discharge limit patient outcomes?

This episode challenges pelvic health professionals to rethink the belief that fast discharge equals great care. Becky argues that symptom resolution and independence are not the same as resilience, capacity, or long-term wellness. By shifting the metric from “how fast” to “how far,” clinicians can deliver truly life-changing pelvic health care.

What you’ll learn in this episode:

(01:51) How insurance-driven models shaped the 80 percent “prior level of function” mindset

(04:16) Why symptom resolution alone does not equal life-changing pelvic health care.

(07:34) How aging and hormonal changes require a long-term, resilient approach to treatment.

(10:37) Why holding patients longer can actually reflect higher clinical skill.

(13:40) How uncovering root causes can expand patient goals far beyond initial symptoms.

(18:00) Why clinicians must help patients envision what is truly possible.

(20:12) How redefining clinical excellence shifts outcomes from short-term fixes to lasting impact.

  • Pelvic Health Clinicians: Stop Treating Only What You See
    2026/01/22

Pelvic health professionals, are you treating what you see or what truly matters to the patient?

In this episode, Dr. Becky Allen challenges pelvic health leaders to reconsider how clinical decisions are made when treatment is based solely on what can be observed. Through a personal case study, she demonstrates how patient history, symptoms, and goals fundamentally change clinical reasoning. The conversation reinforces that true effectiveness comes from understanding before intervening.

What you’ll learn in this episode:

(01:47) How visual assessment alone leads to incomplete clinical conclusions.

(04:51) Why patient history is essential before choosing any intervention.

(12:59) How past trauma can permanently influence movement strategies.

(15:42) Why patient goals must guide assessment and treatment planning.

(18:32) How to differentiate ground-up versus top-down contributors.

(21:40) How to identify the true lowest-hanging fruit in treatment.

(24:43) Why biomechanics mean little without patient context.
