  • Can a High-Drop Running Shoe Be Causing Your Patient’s Leaking?
    2026/03/12

    Dr. Becky Allen answers a DM question about whether higher drop shoes can contribute to urinary leaking, arguing that they can by altering gait biomechanics from the ground up. Shoe drop is defined as the heel-to-toe height difference inside the shoe, with high heels as an extreme high drop and flat flip-flops as zero drop. A higher drop places the foot in relative plantarflexion, limiting dorsiflexion and midfoot pronation, which reduces the normal internal-rotation cascade up the kinetic chain (subtalar motion, tibial and femoral internal rotation) and leaves the pelvis more externally rotated. With less pelvic internal rotation, the pelvic outlet stays smaller and the pelvic floor cannot lengthen and re-ascend automatically to manage pressure, potentially worsening leaking during walking or running. Clinically, Dr. Becky urges assessing footwear, midfoot mobility, dorsiflexion, and rotation mechanics, and suggests working runners from ~10 mm drop toward low/zero drop (e.g., 4 mm and under).

  • The Entrepreneur Identity Shift: Risk, Fear, and Staying in the Arena

  • Stop Managing Pressure. Start Fixing It. A Better Way to Treat Pelvic Floor Dysfunction

    Dr. Becky Allen argues that traditional pelvic floor courses teach “pressure management” the wrong way by focusing on pelvic floor squeezing, Kegels, and pelvic elevation (e.g., bridges) to manage symptoms rather than addressing why pressure symptoms occur. Using a tube-of-toothpaste analogy, she explains that tightening the “lid” (pelvic floor contractions) doesn’t fix excessive downward pressure and may fail if the force is high enough. She presents a framework based on the formula Pressure = Force/Area, emphasizing that to reduce pressure clinicians should decrease force and/or increase the area the force acts upon. She outlines three ways to decrease force: reduce the load (only when appropriate for healing timelines or injury), displace/distribute force by creating more space above the pelvis via rib cage and thorax expansion, and improve force transfer and absorption through the ground-up kinetic chain (midfoot pronation/supination control, ankle dorsiflexion, tibial/femoral internal rotation, and controlled internal rotation throughout). For increasing area, she describes that many patients have a shortened pelvic floor (often posteriorly) and need pelvic floor lengthening—especially under load—which requires pelvic and kinetic-chain internal rotation. She encourages shifting from symptom management to pressure manipulation for lasting resolution and promotes her course waitlist opening February 23rd, which teaches this full framework with modules (foot, ankle, pelvis biomechanics, rib cage/thorax), 70+ exercise videos, and manual therapy techniques.
