First question comes from a 68-year-old man in good overall health:

“Dr. Jacomides, I’ve never had incontinence issues my entire life, but recently I’ve started wetting the bed at night. During the day I have no trouble holding my urine at all. I’ve already tried cutting back on fluids before bed and always emptying my bladder right before sleep, but the problem keeps happening. My PSA and urinalysis both came back completely normal. Is nighttime bedwetting common in men my age, and what are my treatment options?”

Second question:

“I’m a veteran, and back in August my bloodwork before a scheduled cardioversion showed a PSA of 120. My VA urologist gave me some paperwork and referred me to community care through my primary-care doctor, but no one at the VA ever explained what this means or what happens next. I was pretty shocked; nobody had flagged anything like this before. What should I expect moving forward?”

This week, Dr. Mistry and Donna Lee are joined once again by one of our all-time favorite guests, Dr. Declan Fleming! In the first segment, they dive into how cancer diagnoses are changing and why, contrary to what some might think, doctors don’t want patients to get sicker.

Between 1930 and 2006, cancer rates were on the rise—but since around 2006, they’ve started to flatten and even decline. That’s great news! However, there are five cancers still increasing: thyroid cancer, neuroendocrine tumors, stomach cancer, liver cancer, and skin cancer.

One of those rises may actually be linked to the overuse of common over-the-counter medications for GERD, like Prilosec, Pepcid, Tagamet, Protonix, and Nexium. If you’re taking these for more than six weeks, it’s time to check in with your doctor—they can lead to serious complications, even esophageal cancer.

Dr. Mistry and Dr. Fleming also unpack the dangers of fatty liver disease—and how inflammation from excess fat is the real enemy to your body’s health.

In the second segment, the doctors explore an exciting advancement in medicine: the liquid biopsy. Dr. Fleming explains how this cutting-edge tool helps detect and treat cancer more effectively than ever before.

You’ll also hear about some real-life “medical dumpster fires” (yes, that’s an actual term!)—and why Dr. Fleming is often the go-to surgeon to handle them.
