This week’s episode I am going to be discussing Tracheoesophageal Fistula, a congenital malformation that usually occurs during organogenesis and results in an abnormal connection between the esophagus and trachea. TEF occurs when the wall that normally separates the esophagus and trachea does not form correctly before birth. Because the tubes are connected, swallowed food or liquid can be inhaled into the lungs. Who successful performed the first primary repair case of TEF?? What is its pathophysiology? What is the clinical presentation? How do we treat it? Tracheoesophageal fistula (TEF/TOF) is a rare congenital abnormality with incidence approximately 1 per 3500-4000 live births. The separation etiology of primitive trachea and esophagus still not completely understood, the most commonly accepted hypothesis is defect in the lateral septation of the foregut into trachea and esophagus causes TEF.

This week’s episode I am going to be discussing one of our all-time favorites; gastroschisis, the most common type of abdominal wall defect, has seen a steady increase in its prevalence over the past several decades. What does it mean? When was the first case documented?? What is its pathogenisis? How many gastroschisis cases have you transported? Do you have the burning desire to find out what increta/percreta is? Gastroschisis is a congenital anomaly in the rudimentary umbilical ring, most often just to the right of a normally-inserted umbilical cord. There is herniation of a variable amount of viscera through this defect, with no covering membrane or sac. The prevalence of gastroschisis has been steadily increasing, presumably due to environmental factors. Based on the absence or presence of intestinal complications, gastroschisis is classified into simple and complex types

Happy Tuesday crewmembers. This week’s episode I am going to be discussing Commotio cordis. What does it mean? What is the percentage of cases of commotio cordis seen ion ERs today? How on earth does this freakish event occur? Have you ever transported a Commotio cordis case? Do you have the burning desire to find out what Dim mak is and can you really send someone into V Fib by doing it? Commotio cordis, which translates from Latin origin as "agitation of the heart," is defined as sudden cardiac death secondary to a blunt, nonpenetrating anterior precordial impact. Sudden death from commotio cordis is a primary arrhythmic event, and, importantly, the chest blow does not inflict direct structural damage to the heart itself. Despite a lack of awareness, commotio cordis is among the more common causes of sudden cardiac death in young athletes (along with hypertrophic cardiomyopathy and congenital anomalies of the coronary arteries)

Sorry for the late drop. Had a little vacation thois weekend and didn't get a chance to upload the show. This week’s episode I am going to be discussing Congenital diaphragmatic hernias. The presentation. Reviewing the etiology. Pathology. What goes on with the lungs and heart. How it is diagnosed and how to manage it. Congenital diaphragmatic hernia (CDH) is a rare condition with an incidence of 1 in 2,000 to 1 in 4,000 live births. Up to 80% to 95% of cases are diagnosed by antenatal ultrasound with many of the remaining cases diagnosed within the first 24 hours of life. Rarely, patients develop herniation through a diaphragmatic defect beyond the immediate newborn period. In these cases, the diagnosis of CDH may be unsuspected, particularly if the patient has a history of a normal fetal ultrasound and newborn course. This case illustrates one such example and reminds providers to stay vigilant for the diagnosis of late-presenting CDH.

This week’s episode will be about a every important topic that we must deal with. The Airway. Currently, pediatric airway emergencies that require a secure and advanced airway range from 0.6 to 3.3 per 1,000 cases. The median age often varies by institution; however, the highest risk patients commonly are younger than 12 months old. Rapid sequence intubation (RSI) is an airway management technique that induces immediate unresponsiveness (induction agent) and muscular relaxation (neuromuscular blocking agent) and is the fastest and most effective means of controlling the emergency airway. Complications in children occur mainly because of airway management-related events such as difficult approach, laryngospasm, bronchospasm and severe hypoxemia, which may result in cardiac arrest, neurological deficit or death. Have they ever been in a can’t ventilate, can’t intubate situation? Have the Transport Gods ever brought you back to reality? Remember kiddos If A doesn’t happen then B nor C can happen. That’s right boys and girls. We are going to discuss the management of the pediatric critical airway. We will then jump into some RSI knowledge and end the episode with some new approaches and treatments in the wonderful world of pediatric airways.

This week’s episode will be about button battery ingestions. Necrotizing enterocolitis (NEC) is a devastating and unpredictable inflammatory disease. Infants may present with early signs and symptoms and continue on to severe NEC. Other infants may present in severe NEC without any early warning signals. Because of its unpredictable nature the best defense against NEC is a strong offense. Early recognition of the signs, symptoms and risk history by caregivers can mitigate the course of the disease. Unfortunately, most of the early signs mimic other common challenges of the neonate like apnea and temperature instability. So it is the caregiver that must know their patient and subtle changes in the baby’s condition. This show will review some of the causative factors, treatments, complications, prevention, and transport considerations when transporting the NEC patient.
