How did a document printed days earlier, and supposedly obsolete, end up in a place with no logical path or visible entry?

    In this episode, we follow the sequence of people, records, and decisions that created the puzzle: a passenger who printed a pass, an airline that canceled a flight without public notice, a maintenance key ledger, and the contractor who found the pass.
