Hi, I'm Gabrielle, a registered nurse who has specialized in caring for medically complex patients and their families for over 25 years; including those in intensive care, oncology, emergency medicine and many places in between. I'm also a Christian, a mom, a granny, an animal lover and I am fueled by sunshine!

Over the years, I’ve witnessed the emotional turmoil, confusion, and uncertainty individuals and families face when making decisions for themselves or for the people they love; whether it’s navigating a serious illness, choosing treatment options, considering hospice care, or facing end-of-life decisions. These moments carry enormous weight.

My goal is to help you feel informed, supported and less alone during one of the hardest and most pivotal times of your life.

I believe in having the hard conversations that no one wants to have because clarity creates confidence, and making informed decisions creates peace.

I truly believe that I’ve been called to do this work and it is an absolute honor to walk alongside you.

    In this episode, I’m talking about polypharmacy and why knowing exactly what medications and supplements you or your loved one are taking is an important part of patient advocacy. I’ll share a real-world example of how medication side effects can mimic other medical conditions, potentially leading to additional testing and confusion when the underlying issue may be hiding in the medicine cabinet. We’ll talk about keeping an accurate medication list, regularly reviewing medications with your healthcare provider or pharmacist, using one pharmacy when possible, recognizing a potential prescribing cascade, and simplifying medication routines when appropriate. Because medication safety isn't just about taking your medications as prescribed—it’s also about understanding what you’re taking, why you’re taking it, and how those medications may be affecting you.

    Create a Comprehensive Medication List

    Your medication list should include:

    • Prescription medications
    • Over-the-counter medications
    • Vitamins
    • Herbal supplements
    • Inhalers, patches, creams, liquids, and other medications
    • Medication strengths and doses
    • How and when each medication is taken
    • The reason each medication is being taken
    • Medication start dates and dose-change dates
    • Allergies and previous negative medication reactions
    • Emergency contacts
    • Prescribing provider information
    • Pharmacy information

    Step 1: Create a current, comprehensive medication list for yourself and/or your loved one. Include every prescription, over-the-counter medication, vitamin, herb, and supplement being taken.

    Step 2: Bring that medication list to every doctor's appointment, emergency department visit, and hospital admission. Ask your healthcare provider or pharmacist to regularly review the list with you.

    Step 3: Become an active participant in medication safety. Ask questions about why each medication is being prescribed, what side effects to watch for, whether medications may be interacting with one another, and whether there are opportunities to simplify the medication routine.

    Remember: Your medicine cabinet tells a story about your health. Make sure you—and the people caring for you—know what that story says.

    Patient advocacy starts with knowing what you're taking.

    Ask questions.

    Speak up.

    And don't assume every new symptom means there's a new diagnosis. Sometimes, the answer may be hiding in the medicine cabinet.

Having these conversations won’t prevent the pain, grief, or uncertainty of a medical crisis—but it can give your people clarity, confidence, comfort, and peace when they need it most. This conversation may be uncomfortable, but it is one of the most selfless things you can do for the people you love.
