    Should you get another one?

    No. And this answer is coming from someone who loves certifications, creates them, and sells them.

    In this Q&A episode, Dr. Z answers three questions straight from practitioners — why certifications alone won't grow your practice, whether it's ever okay to fire a patient, and how to know when you've ordered enough labs versus when you're just ordering out of fear.

    In this episode:

    • Certifications live on the fulfillment side of the triangle. Marketing and sales are the other two sides. If you don't have people coming in the door, it doesn't matter how brilliant you are at solving the medical mystery. There's nobody there to help.

    • You're not going to just focus on marketing. You're going to master marketing, sales, and fulfillment. You're the CEO of your practice. You have to know enough to direct it even when other people are doing it.

    • Yes, it is okay to fire a patient. Dr. Z has done it more than once. The reason was always the same — mistreating clinical staff. It might feel like money walking out the door. It's actually the most important message you can send your team: I will back you up.

    • Labs: only order what's going to change the care plan. If it's not going to make a difference to clinical decision-making, it doesn't go on the order. Involve the patient in the discussion, prioritize the list, and explain your reasoning. A ranked list of five tests where they choose three is better than surprising them with a $2,000 bill.

    • The activist patient needs connection, presence, to feel truly heard. If you walked in distracted, moving fast, barking orders — they're not signing up. Hard sales tactics and ego are instant turn-offs.

    • The artist patient wants uniqueness and a personalized approach. Tell them you do it the same way with everyone and watch them disappear. The scholar is the only one who actually wants every detail — and they're going to need time, data, maybe a spreadsheet, before they decide.

    • The faster decision-makers are class president, cheerleader, and actor. The slower ones are activist, artist, and scholar. Know which one is sitting across from you — and adjust.

  • No, the FDA Did NOT Just Approve Peptides

    Everyone's excited. The FDA is meeting and they're going to approve peptides.

    Except that's not what's happening.

    The organization that gives recommendations to the FDA is meeting. The FDA has already stated they are not promoting or on the side of approving these compounds. And Alabama just became the first state to formally prohibit prescribing, administering, and dispensing research grade peptides — with a pointed reminder that research grade patient waivers offer zero legal protection.

    This episode isn't really about peptides. It's about what happens when science, patient demand, and social media outpace regulation — and what you're going to do in your practice when that gap keeps widening.

    Learn all about:

    • Alabama's Board of Medical Examiners issued a formal joint notice in May 2026 making the state the first to formally prohibit research-grade peptides in practice. The warning that research-grade patient waivers offer no legal protection is the part every prescriber needs to sit with.

    • Peptides currently live in a gray zone — not a prescription, not a supplement. Access is not the same as authority. Authority is not the same as legal protection. If you're a prescriber using peptides in practice, an IRB is currently the only approved route. Know your state.

    • Dr. Z uses peptides personally. She also doesn't do things in her practice unless she has full legal authority to do it. Those two things can both be true at the same time. You don't have to be the test case.

    • The bigger issue isn't peptides specifically. It's what happens when you have the license, the training, the experience, and a patient demanding care that regulation hasn't caught up with yet. Where do you land on that?

    • The story of the physician who was medically gaslit in the ER after his own colonoscopy complication — dismissed because his pulse didn't hit the standard trigger, even though as an athlete his elevated pulse was meaningful. The nurses saved his life. The experience cost him his career and his practice.
