Dr. Jacob Teitelbaum joins Dr. Rodger Murphree to explain why chronic pain is often more complicated than a single injury, diagnosis, or abnormal scan. Drawing on five decades of treating people with chronic fatigue syndrome, fibromyalgia, and persistent pain, Teitelbaum describes four major biochemical drivers of pain: muscle pain, inflammation, nerve pain, and the brain changes that can develop when pain becomes chronic. He says modern diet, poor sleep, nutritional deficiencies, diabetes, environmental stressors, and other factors can contribute to these overlapping pain pathways. Teitelbaum explains his SHINE approach—sleep, hormones, infections, nutrition, and exercise as able—and describes how inadequate energy production may leave muscles shortened, tight, and painful. He discusses the importance of restorative sleep, thyroid and hormonal support, nutrition, and appropriate exercise, along with dietary changes aimed at reducing inflammation. He and Murphree also discuss omega-3 fatty acids, curcumin, Boswellia, CoQ10, red ginseng, and other nutritional approaches Teitelbaum uses as part of a broader strategy for improving energy and reducing pain. The conversation also examines neuropathy and the growing connection Teitelbaum sees between diabetes, vitamin B12 deficiency, and nerve pain. He discusses metformin, GLP-1 medications, alpha-lipoic acid, and nutritional support before explaining how pain lasting longer than six weeks may involve microglial activation and other changes in the brain. Teitelbaum discusses PEA and low-dose naltrexone as approaches he uses for this component of chronic pain, while emphasizing that prescription medications can be valuable when used appropriately alongside other treatments.
