- The envelope’s bar code and tamper strip were intact and logged as never loaded onto the pallets-removed from a secondary storage batch before the run.
- Investigators found uniform four-inch mud settled around the grate with no footprints within six feet, and the signal shed’s stop-request transmitter showed no visible damage and all relays green.
