    About The Episode:

In this episode, Marc discusses the upcoming midterms. Democrats are well-placed to win the House, but can they take the Senate too?

    Key Takeaways:

The Republicans look increasingly vulnerable in the November midterms.

There is an almost lock for Democrats to win the House and their projected majority is going up.

That gives them comfort on bill passage.

Markets give Democrats about a two-thirds change on winning the Senate.

To me, the Senate looks more like a toss-up, but Democrats are clearly gaining.

I have revised my Senate map to no better than 51 for the GOP and that could even be 50 – just enough to maintain control because the White House is Republican.

But in a few weeks Democrats could continue to gain.

NC looks sure to flip to Democrats. AK and OH are possible for Democrats as well.

One more flip and they win control.

Medicaid and Exchange cuts are putting providers at risk.

    About The Episode:

In this episode, Marc discusses that coming Medicaid and Exchange cuts are putting providers at risk. Any healthcare reform changes need to be thoughtful.

    Key Takeaways:

We clearly need substantive healthcare reform, including price reform.

But existing changes to Medicaid and the Exchanges are putting providers at risk.

The vast majority of dollars flow through the health system to providers for care.

Major reductions in coverage will drive the uninsured rate up dramatically and impact provider revenue and bottom lines.

Affordability is worsening, further impacting providers.

Tenet and HCA are already seeing impacts from Exchange enrollment reductions.

    Non-profits will fare far worse.

The magnitude of Exchange and Medicaid cuts have yet to be felt.

Rural healthcare is especially fragile and is at greater risk with the cuts.

A temporary $50 billion fund will not save these providers.

  • 147. Fighting the Wrong War: Study Casts Aside Insurers as the Affordability Bogeyman
    2026/10/02

Overall costs, not insurer profits, are causing premiums to explode.

    About The Episode:

In this episode, Marc discusses a new study that shows overall costs, not insurer profits, are causing premiums to explode.

    Key Takeaways:

There is a familiar story in American healthcare: health insurance companies are the reason healthcare costs so much.

But a new study shows that overall healthcare costs not insurer profits drive premium increases.

Researchers looked at commercial insurance data from 2011 through 2024.

Average premiums increased by 78.4% during that period — an increase of $3,143 per enrollee.

But underlying healthcare spending increased by $2,844.

Thus, 91% of premium growth was associated with growth in healthcare spending.

Just 9% went toward insurer markup.

Medical loss ratio requirements limit insurer margins.

During the period studied, insurer markup (admin expense and margin) fell from

    18.6% to 14.9% in 2024.

    Certainly, make insurers more accountable.

But the current political argument over insurer greed keeps us from focusing on the real issue – the need for real healthcare reform: Price reform, pivoting to primary care and care management, and real affordable access.
