Episode Twenty-Two — The Investigation of the Investigation

Peter Sutcliffe had been caught.

The trial was over.

Twenty life sentences had been imposed.

But another investigation was only beginning.

This time, Peter Sutcliffe was not its subject.

The police were.

In December 1981, Sir Lawrence Byford delivered his examination of the police investigation that had failed, for more than five years, to identify and arrest Peter Sutcliffe.

His task was not to investigate the killer.

It was to investigate the investigation.

In this episode, we step inside the enormous machinery of the hunt itself — its leadership, its Major Incident Room, its information systems, the extraordinary influence of the Wearside Jack letters and tape, and the warnings that were raised while the investigation was still underway.

But first, we remove something that investigators at the time never possessed:

The answer.

By stripping away hindsight before putting the evidence back, we examine what those decisions actually looked like at the time — and why Byford ultimately concluded that Peter Sutcliffe should have been identified and arrested earlier.

Through hindsight bias, confirmation bias, belief perseverance, cognitive overload and escalation of commitment, we explore how experienced, hardworking people can operate within a system that is struggling to connect what it already knows.

Because this was not simply a story of missed clues.

It was a story of information without understanding.

Warnings without sufficient change.

Enormous effort without effective coordination.

And an institution forced to confront an extraordinarily difficult question:

How had one of the largest murder investigations in British history possessed so much information about the man it was hunting...

And still failed to see him?
