Rick turns himself into a pickle to dodge a family therapy session, but when Beth pockets the restoration syringe, he's stranded, washed into the sewer, and forced to build rat-and-cockroach exoskeletons just to survive. He eventually escapes into a foreign government facility, fights his way out, and limps into the therapist's office covered in blood — while Beth, Summer, and Morty have already spent the session being quietly dismantled by Dr. Wong.
The episode uses Rick's absurd survival arc as a counterpoint to the therapy scenes, where the real damage is exposed: Rick engineered the pickle stunt for the same reason Beth defends him and the marriage fell apart. Dr. Wong's argument — that maintaining things is just unglamorous work, not an adventure — is the one Rick cannot answer, and the one the show lets land without deflating it.
