PEPFAR saved an estimated 26 million lives. It was also built so fast, and so far from the countries it served, that when the money froze in 2025, much of it was left standing on nothing.

In the second half of this conversation, Michael Brown turns to the other side of the ledger with John Donnelly, author of Lifeline. Who owned PEPFAR: African leaders, or just African staff? What did a 22-year emergency cost, and who paid? And can a five-year handover on Washington’s clock succeed where 20 years of planning didn’t?

In this episode:

  • Was PEPFAR African-authored or only African-staffed? The answer depends on the country, from Nigeria’s Harvard-partnered program in Jos to the top-down rollout in western Kenya.
  • Why Donnelly calls 22 years of emergency footing “ridiculous,” and why the first five years should have included a transition plan.
  • How big the program really was: in Kenya, PEPFAR made up most of the US embassy budget, and in Nigeria it grew from $62 million to $960 million.
  • What happened in Nigeria when PEPFAR pulled back, and whether it built the capacity to run a program but never to pay for it.
  • Inside the State Department today: 35 country agreements, about $14 billion over five years, and which countries (Zimbabwe, Ghana, South Africa) haven’t signed on.
  • The opportunity cost question: what a single-disease focus means for the child with sepsis in the next bed.
  • Whether PEPFAR bought lasting goodwill in Africa, and how it compares with China’s growing influence.
  • Why no one can say how many people have died since the 2025 funding freeze.
  • If Donnelly could start over today: the case for a strategic plan with the handover built in from day one.

Michael closes with his verdict on a program that both its builders and its wreckers failed to imagine ending: “Both sides failed the same way.”
