Oncocytic thyroid carcinoma, historically classified as a Hürthle cell variant of follicular carcinoma, represents a distinct clinical entity driven by mitochondrial DNA mutations and widespread chromosomal loss of heterozygosity. Pathologically defined by over 75 percent eosinophilic, mitochondria-dense cells, it exhibits an aggressive phenotype with heightened rates of distant metastasis and structural recurrence. Crucially, metabolic mitochondrial dysfunction leads to loss of sodium-iodide symporter expression, rendering these tumors intrinsically resistant to radioactive iodine therapy despite high fluorodeoxyglucose avidity on PET imaging. Management prioritizes complete surgical resection, with systemic kinase inhibitors indicated for progressive, refractory disease.
