The Johns Hopkins University GWEP collaborates with the Schools of Nursing at the University of Maryland and Morgan State University to integrate the 4Ms Framework into nursing home care for both undergraduate and graduate nursing students.
This initiative includes an interprofessional didactic component followed by a clinical experience. Students learn to facilitate advance care planning discussions and explore what matters to each resident using an “All About Me” booklet and the development of personalized storyboards. Students also receive training in medication safety, medication reconciliation, and mobility and fall risk assessment.
