- Dale Hayes had held a pharmacist’s license for 31 years with no disciplinary marks and had delivered those capsules to William Gray on at least four occasions before the jar was found.
- A cloth-bound notebook discovered in an assisted living facility 25 years later linked seven names from the notebook to death certificates recorded as cardiac arrest in counties where Hayes had worked.
