The same alarming rhythm can lead to very different actions. Nora delivers a rapid-fire adult NCLEX-RN cardiac-rhythm review with six original cases: monitor artifact, ventricular tachycardia with and without a pulse, symptomatic bradycardia, heart block, atrial fibrillation and pacing capture. Practise strip arithmetic, distinguish defibrillation from synchronized cardioversion, avoid the adenosine and torsades traps, and test yourself in a final recall round. Current AHA 2025 algorithms and primary source notes accompany the episode.
- Determine pulse and perfusion before choosing a rhythm response; VF/pulseless VT are shockable, while PEA/asystole are not.
- Unstable monomorphic tachycardia with a pulse usually requires synchronized cardioversion; sustained polymorphic VT requires unsynchronized shock.
- Recognize high-grade AV blocks, verify mechanical pacing capture, and distinguish AF rate/rhythm control from stroke prevention.
