  • Episode Thirteen: Caring for the Caregiver- Grand Rounds- Sepsis Algorithm
    2026/10/20

Caring for the Caregiver- Grand Rounds- Infant Emergencies - Neonatal Sepsis Risk/Nurse Duty of CareMy role is to empower and to protect the caregiver first, as mentor on the journey to self actualization and second to create a human shield, to challenge each caregiver to care for themselves health, heart, and soul. Critical Ground Rounds: Sepsis Calculator is a critical clinical tool used to assess the probability of early onset sepsis (EOS) in newborns based on maternal risk and clinical presentation. Nursing duty of care requires comprehensive assessment, continuous monitoring, timely reporting, and detailed documentation to ensure patient safety and provide a legal shield. Collaboration between nurses, pediatricians, and neonatologists is essential for escalating care and reducing delays that could lead to adverse outcomes. Do we stop to collaborate on personal care. Do we stop to collaborate on health and wellness. This series is a step in that direction, to challenge, to question, to uncover, and to grow personal and internal "wealth" as a caregiver. As we grow the tree, grow life. One branch at a time. One life at a time.

  • Episode Ten: Roth Conversion Myths
    2026/10/08

Today, we’re looking at a decision that asks us to think beyond this year’s tax bill.

As we approach age 73, we learn that A Roth conversion moves money from a traditional retirement account into a Roth IRA. The taxable portion generally adds to income in the year of conversion. In return, converting may reduce future required minimum distributions from traditional accounts. Roth IRAs do not require distributions during the original owner’s lifetime.

The question is whether paying tax now could improve the *lifetime* result. That takes careful planning around the amount converted, tax brackets, other income, and future needs. A conversion deserves a calculation, not a slogan.Who Will Benefit from a Roth Conversion

A Roth conversion is most advantageous when your individual circumstances allow you to maximize long-term tax-free growth while keeping immediate tax costs low:

      • Investors in a lower tax bracket today than in retirement:
      • If you expect your marginal tax rate to be higher in the future (due to higher retirement income, loss of deductions, or tax law changes), converting now locks in today's lower rate.
      • Early retirees in "gap years":
      • The window between leaving your career and starting Required Minimum Distributions (RMDs) or Social Security (typically between ages 60 and 73) provides a low-income opportunity to execute partial conversions and "fill up" lower tax brackets.
      • Savers with outside cash to pay the tax:
      • Paying the conversion tax from a non-retirement savings or taxable brokerage account leaves 100% of your converted IRA balance inside the Roth to compound tax-free.
      • People with long time horizons:
      • Having decades before needing withdrawals allows tax-free investment compounding to easily outpace the initial tax hit.
      • High earners using "Backdoor" or "Mega Backdoor" strategies:
      • High-income earners barred from direct Roth contributions can use nondeductible IRA or after-tax 401(k) contributions to build tax-free retirement wealth.
      • Those seeking to eliminate lifetime RMDs and benefit heirs:
      • Original Roth IRA owners have no RMDs during their lifetime. Under the SECURE Act, non-spouse heirs must fully distribute inherited IRAs within 10 years; inheriting a tax-free Roth IRA prevents forcing heirs into high tax brackets during their peak earning years.
      • Accounts with nondeductible (after-tax) basis:
      • If your traditional IRA contains after-tax contributions, only the earnings portion is taxed upon conversion, lowering your overall tax costI found that converting pre-tax retirement funds into a Roth IRA generates taxable ordinary income in the year of conversion but eliminates required minimum distributions (RMDs) during the original owner's lifetime and allows future growth and withdrawals to be completely tax-free.

    Key themes:

      • RMDs cannot be converted:
      • Under IRS regulations, required minimum distribution amounts are ineligible for rollover or conversion. You must satisfy your full annual RMD for the year across all traditional IRAs before converting any remaining pre-tax funds.
      • Tax bracket impact & bracket filling:
      • Converted funds are treated as taxable income, which can push you into a higher federal tax bracket. A common strategy is executing partial conversions over multiple years to "fill up" lower tax brackets without crossing into higher marginal tiers.
      • Secondary tax considerations:
      • Added conversion income can increase the taxable portion of Social Security benefits, trigger Medicare Part B/D premium surcharges (IRMAA), or trigger the 3.8% Net Investment Income Tax (NIIT).
      • Lifetime RMD exemption:
      • Once funds are successfully inside a Roth IRA, they are exempt from RMD requirements during your lifetime, giving you full control over when to take withdrawals.

    2026/10/08

Strategic Roth Conversions: What Are We Talking About Today?

As we approach the age when required minimum distributions (RMDs) begin, it is worth looking beyond this year’s tax bill.

A Roth conversion moves money from a traditional retirement account to a Roth IRA. The taxable portion generally increases income in the year of conversion.

Converting some of the balance may reduce future RMDs from traditional accounts; a Roth IRA has no lifetime RMDs for its original owner.

The age at which RMDs begin depends on your birth year, and an RMD due for the current year cannot itself be converted.

Could paying tax now improve the lifetime result? The answer depends on the conversion amount, current and future tax rates, other income, and the money needed to pay the tax.

Let’s do the calculation and let us learn together.

  • Episode Seven: Creating the Standard of Care for Your Own Life
    2026/10/01

Every single day, you walk into an environment where the 'Standard of Care' is non-negotiable.

As physicians, advanced practice providers, clinical leaders, healthcare business owners, and nurse professionals you hold human lives in your hands. You make high-stakes, split-second decisions. You manage clinical protocols, oversee practice overhead, lead teams, and bear the weight of intense responsibility. But behind the white coat, behind the title, and behind the clinical success many of you are living with a quiet contradiction. You are world-class at diagnosing, stabilizing, and saving everyone else—yet no one ever taught you how to care for yourself. Nobody taught you how to protect your own earning power. Nobody handed you a blueprint for auditing 'silent' professional debt, building household margin, or safeguarding your family when crisis hits. You were trained for self-sacrifice, not self-stewardship. And so, you carry the private stress of exhaustion, white-coat burnout, and disorganization behind closed doors.

    Today, that changes.

Today, we are establishing a new clinical protocol. We are taking the exact same rigor, diagnostic precision, and uncompromising excellence that you bring to bedside patient care, and we are applying it inward.

Welcome to The Attard Methodology™—where we establish the Standard of Care for your own life, your household, and your professional legacy. Let’s begin."
