  • The Pilot Sucked Out of the Cockpit
    2026/09/23
At 17,000 feet, British Airways Flight 5390 suddenly became a fight for survival when the cockpit windscreen shattered, violently ejecting the captain from his seat. With the aircraft still in the air, the co-pilot took control while the cabin crew desperately held the injured captain in place, preventing him from being completely lost from the aircraft.
Against extraordinary odds, the crew managed to bring the aircraft safely down at Southampton. Miraculously, the captain survived the freezing temperatures, extreme pressure, and devastating physical trauma.
The investigation later revealed that the cockpit window had been secured with the wrong-sized bolts during maintenance. What appeared to be a simple maintenance mistake exposed deeper problems involving procedures, human factors, and workplace pressures.
This incredible near-disaster became a powerful lesson in aviation safety, highlighting the remarkable courage of the crew and the importance of understanding how human error and systemic failures can combine to create catastrophic risks.

  • Boeing's fatal gamble on Flight 811
    2026/09/22
In 1989, United Airlines Flight 811 suffered a catastrophic explosive decompression after its forward cargo door suddenly blew open at cruising altitude, tearing a massive section from the aircraft and killing nine passengers.
What followed was a heartbreaking fight for the truth. The parents of victim Lee Campbell refused to accept the initial explanation and began their own relentless investigation into what had caused the disaster. Their search uncovered serious questions surrounding a known design flaw in the aircraft’s cargo-door locking system and the delays in addressing it.
As the investigation continued, evidence challenged the original conclusion that human error was responsible. The tragedy ultimately exposed how a preventable mechanical defect and delayed safety action could have devastating consequences.
This is the story of Flight 811, the families who refused to stop searching for answers, and the painful lessons it revealed about aviation safety when known risks are not addressed in time.

  • How 297 People Survived Air France 358
    2026/09/21
Air France Flight 358 crashed while attempting to land at Toronto Pearson International Airport in August 2005 during a severe thunderstorm. The Airbus A340 touched down on a rain-soaked runway but failed to stop, eventually overrunning the runway and crashing into a ravine.
The aircraft was destroyed by a massive post-crash fire, but remarkably, all 307 people on board survived. The rapid evacuation of the passengers and crew played a crucial role in preventing a far greater loss of life.
The investigation found that the aircraft touched down farther along the runway than it should have, leaving insufficient distance to stop on the slippery surface. The crew also faced rapidly changing weather conditions and delays in applying maximum deceleration measures, contributing to the runway overrun.
The accident highlighted the dangers of landing during severe storms, particularly when heavy rain, wind changes, and limited runway safety areas combine to leave little margin for error. It also reinforced the importance of precise landing decisions, effective crew training, and rapid emergency evacuation procedures.
