- Armstrong applied in December 2008 claiming a divinity degree and submitted a reference letter signed by a Reverend Ryan Cord, who does not exist.
- Hiring coordinator Jean Shattuck called the phone number on the reference and received confirmation, and did not verify seminary records.
- A nurse, Connie Marsh, noted in spring 2017 that a certification number returned an error and filed the note without follow-up.
