    Today's Case A 56-year-old male presents to the emergency department with complaints of fevers and chills for the past 3 weeks. Other review of systems is negative for headache, cough, shortness of breath, abdominal pain, dysuria, urinary frequency or urgency, or back pain. Vital signs reveal his temperature is 102.4°F, pulse rate is 118/min, blood pressure is 154/80 mm Hg, and respiration rate is 17/min. Physical exam is normal except for tachycardia, but otherwise regular rhythm without rubs or murmurs. The emergency room physician requests admission for the patient for sepsis secondary to suspected pyelonephritis based on the abnormal urinalysis. The symptoms began approximately 1 hour earlier during dinner with family when her speech became slurred and incomprehensible. Due to her difficulty speaking in the ED, she is unable to provide additional information. Per her husband, her past medical history is notable for coronary artery disease, prior heart attack, diabetes mellitus, hypertension, breast cancer, and low back pain.
