While the patent itself was for the invention of perforated toilet paper, not the preferred hanging orientation, the illustration is the primary reason many people argue that "over" is the original and intended method.

Cottonelle – Over or Under?
Cottonelle reports that approximately 70% of people prefer the "over" orientation. Their research outlines the advantages and disadvantages of both "over" and "under," noting that around 20% of people become annoyed when the toilet paper is hung the "wrong" way. They also found that many people admit to quietly changing the orientation of the roll when visiting someone else's home.
