- A laminated white card reading "ACCOUNT SETTLED - WARD." with a seven-digit invoice number was photographed three times by meter reader Brenda Farrell on August 9, 2011.
- Harwick Services, owned by Thomas Ward, supplied linen and delivered to Riverside District bars on Tuesday and Friday routes.
- From 2005 through summer 2011 the seven Riverside District bars recorded thirty-five clean license renewals across five renewal cycles with zero holds.
- Citywide norms expected at least twelve inspection holds for comparable bars over the same five-year period.
- Eugene Snyder, chief field supervisor for the Department of Buildings and Inspections, had assignment authority and allegedly routed inspections to two controlled inspectors while taking payments from Ward since spring 2005.
